If you are a new functional medicine patient, please select this option. Your initial functional medicine visit will generally include a detailed history, exam, suggestions for next steps regarding lab work or additional diagnostic testing. In addition, Dr. Emily may make some initial personalized diet and lifestyle recommendation. You will receive intake paper via email before your appointment. Please be certain to complete and submit this important paperwork before your scheduled appointment.
